Family Resource Center Case Manager

Overview Position: Family Resource Center (FRC) Case Manager – GLIDE, San Francisco, CA. The FRC provides trauma-informed family engagement, intensive case management, family advocacy, and crisis intervention for families in the Tenderloin. Collaborates with GLIDE program staff, schools, community organizations, and Protective Service Workers. Regular and predictable attendance is essential and non-traditional hours (early evenings, holidays, weekends) may be required. Base pay $31.00/hr - $31.50/hr Responsibilities Using bilingual language fluency, conduct intake assessment, surveys and enrollment, collect and provide information on community resources and GLIDE services, provide advocacy and coordination to reduce barriers to services, find creative solutions to meet immediate and ongoing needs, and ensure linkage and support to promote health, recovery, and wellness for children, youth, and their families. Determine eligibility and review/assist with applications for entitled benefits and financial resources including, but not limited to, General Assistance, CalFresh, MediCal, Social Security Disability, emergency rental assistance, and low-cost housing programs. Develop and attend FRC outreach events and build rapport and working relationships to reduce barriers to community resources and improve awareness of GLIDE programs. Collaborate with other providers from community agencies, GLIDE program staff, interns, and volunteers to ensure comprehensive and coordinated services. Facilitate and co-facilitate parent groups, specifically for expecting parents and parents with newborns. Lead and facilitate FRC’s weekly Mommy/Daddy & Me group. Deliver services in the community, when contracted, and at school meetings. Manage documentation in a timely manner, including enrollment, assessments, individual/family service plans, follow-up contacts, CFS reports, program evaluations, and service entries in Salesforce and other databases. Participate in regular staff meetings, trainings, supervisory meetings, and adhere to agency policies, procedures, and professional ethics. Essential Duties & Responsibilities Using bilingual proficiency, conduct intake assessments, surveys and enrollment, provide information on resources, advocate and coordinate services, and support health, recovery, and wellness for children, youth, and families. Determine eligibility and assist with applications for benefits and housing programs. Develop and attend FRC outreach events to improve program awareness. Collaborate with providers, GLIDE staff, interns, and volunteers to ensure coordinated services. Facilitate parent groups for expecting parents and parents with newborns; lead the weekly Mommy/Daddy & Me group. Deliver services in the community and at school meetings when contracted. Manage documentation related to enrollment, assessments, service plans, follow-ups, and program data. Engage in staff meetings, trainings, and uphold agency ethics and policy. Qualifications Must be fluent in English and Spanish (reading and writing) to address the needs of the targeted population. Ability to prepare correspondence, reports, assessments, service plans, and progress notes in both English and Spanish. Ability to convey and obtain accurate information respectfully, including with individuals who have language barriers or disabilities. Knowledge of comorbid and chronic mental health disorders (e.g., depression, anxiety, trauma, addiction, grief/loss, psychosis). Commitment to strength-based values and recovery-oriented approaches. Strong attention to detail, organizational skills, and time management. Ability to work independently and collaboratively; capable of building cooperative relationships with professionals and community members. Adaptability to cultural, linguistic, political, and community contexts; sensitivity to risk and resilience factors. Adherence to laws, ethics, and values ensuring effective and efficient care. Commitment to GLIDE’s mission and values. Education and/or Experience Bachelor’s degree in psychology, social work, or related health and human services field with 2+ years of relevant experience. Experience working with children, youth and families in a community-based social service setting. Understanding of client communities and bio-psycho-social-spiritual and economic factors affecting families/clients. Experience developing and leading workshops and activities for families/adults. Knowledge of local, state, and national resources, including services for families with minor children. CPR and First Aid training (or willingness to obtain); tuberculosis screening; immunization proof (pertussis, influenza, MMR). Work Environment GLIDE’s buildings are located in the Tenderloin; open-door policy for community and clients. Physical Requirements Ability to work on a computer and see details at close range; normal hearing and ability to communicate effectively. Finger dexterity for standard office equipment; ability to sit/stand and navigate office spaces. This is a full-time (40 hour/week) non-exempt position.
