Licensed Practical Nurse (LPN) – Home Health Location: Employment type: EMR: Role Overview Provide in‑home skilled nursing under RN/physician direction: medication administration, wound care, patient teaching, and safety monitoring. Support timely, accurate documentation to ensure continuity of care. Key Responsibilities Perform LPN scope interventions during scheduled home visits and promptly report changes in condition. Implement the established Plan of Care; reinforce disease‑specific and medication teaching. Obtain vitals, administer treatments, and perform wound/skin care per order and policy. Document each visit in WellSky within required timeframes; maintain supply control and infection prevention. Collaborate with RN Case Managers and the interdisciplinary team to achieve quality outcomes. Qualifications Graduate of an accredited program; Active Illinois LPN license; BLS/CPR. 1+ year nursing experience; Medicare home‑health experience preferred. EMR experience (WellSky a plus); strong communication and customer‑service skills. Valid driver’s license, auto insurance, and reliable vehicle.
