About the Role

As an RN Case Manager, you'll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You'll collaborate with an interdisciplinary team to ensure every patient is cared for - body, mind, and spirit. With a strong focus on clinical judgment, communication, and organizational skills, you'll be responsible for:

• Assessing patient and family needs and developing personalized care plans
• Delivering skilled, compassionate nursing care and symptom management
• Administering medications and coordinating care across the healthcare team
• Educating and supporting families to promote comfort and peace of mind
• Leading and guiding hospice aides to ensure consistent, compassionate care
• Maintaining accurate documentation and participating in ongoing education

What You'll Bring

• Current RN license in Virginia (multi-state or single-state)
• CPR certification and at least two years of nursing experience (hospice or home health preferred)
• Strong clinical judgment, communication, and organizational skills
• Reliable transportation, valid driver's license, and insurance
• A self-directed, compassionate spirit ready to serve your community
