This role is responsible for planning, organizing, and directing hospice care for patients and families, with a strong emphasis on community-based care and patient-centered outcomes.The ideal candidate is clinically skilled, compassionate, and self-directed, with a strong ability to manage complex patient needs while providing emotional support to patients and families during sensitive times.Essential Duties and ResponsibilitiesComplete initial, comprehensive, and ongoing patient and family assessments to determine hospice needsProvide professional nursing care utilizing the nursing process, including assessment, diagnosis, planning, intervention, and evaluationDevelop and implement individualized care plans focused on palliative care, incorporating patient and caregiver goalsMonitor patient condition and symptom management, revising care plans as necessaryAdminister medications and treatments in accordance with physician ordersEducate patients and caregivers on care plans, medications, diet, and self-care techniquesAssist patients with activities of daily living and promote comfort and quality of lifeMaintain accurate, timely clinical documentation, including pain and symptom management in the EMRCommunicate changes in patient condition to providers and obtain/document physician orders as neededCollaborate with interdisciplinary team members to ensure coordinated, holistic careSupport emotional needs of patients and families, promoting dignity and comfortParticipate in interdisciplinary team meetings and care planning discussionsCoordinate medical equipment and additional services required for patient careSupervise hospice aides and delegate responsibilities appropriatelyParticipate in on-call rotation and respond to patient needs as assignedEnsure a safe and supportive environment for patients and familiesEngage in ongoing professional development and continuing educationSupport quality improvement initiatives and organizational goalsTravel throughout the service area as required.Education and ExperienceMust be licensed or eligible for licensure as a Registered Nurse in Virginia (multi-state compact or Virginia-only license)Current CPR certification requiredMinimum of two years of nursing experience preferred, with at least one year in public health, home care, or hospiceCritical care experience preferredStrong clinical assessment, critical thinking, and problem-solving skillsAbility to manage high-acuity patients with frequent medication adjustmentsUnderstanding of pain management principles and end-of-life careExcellent verbal and written communication skillsAbility to work independently with minimal supervisionDemonstrated ability to prioritize and manage workload effectivelyMust be able to provide compassionate care and remain composed during crisis situationsValid driver's license and reliable, insured transportation requiredPhysical / Sensory / Cognitive RequirementsAbility to stand, walk, bend, kneel, reach, and lift patients or equipment up to 50 poundsAbility to remain in physically demanding positions for extended periodsVisual acuity sufficient to assess patient condition and read medical instrumentsHearing ability sufficient to communicate with patients, families, and team members and respond to alarmsAbility to perform essential nursing tasks, including lifting, positioning, and transferring patientsCapability to meet the physical demands of home-based patient care environmentsYour BenefitsHealth, Dental & Vision InsuranceRetirement Plan with Company MatchPaid Time Off (PTO)Paid Volunteer TimeThrift Shop DiscountVoluntary Benefits, including Critical Illness coverageBlue Ridge Care is a non-profit, equal opportunity employer.
