This position is responsible for resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams. The ideal candidate is compassionate, clinically skilled, organized, and passionate about enhancing the lives of seniors through exceptional care and teamwork.
