Work Environment

Our facility is situated in the vibrant city of Jacksonville, known for its diverse population and beautiful surroundings. The rehabilitation center is a hub for outpatient therapy services, where our team collaborates closely. The work schedule is Monday to Friday, from 8:00 am to 5:00 pm, allowing for a balanced professional and personal life. Key Responsibilities

• Conduct comprehensive evaluations to assess patient conditions through detailed history reviews and specific diagnostic assessments, developing tailored treatment plans based on your findings.

• Communicate assessment results and active treatment plans to patients and their families, ensuring they are engaged in the goal-setting process for their rehabilitation.

• Implement treatment protocols with a focus on evidence-based practices, continuously assessing and modifying the approach based on patient progress and feedback.

• Collaborate with interdisciplinary healthcare teams to discuss treatment strategies and modification of plans based on evolving patient needs.

• Document care activities accurately and promptly, including evaluation notes, daily encounter records, interim progress summaries, and discharge evaluations.

• Exhibit time management skills by maintaining a full patient schedule while meeting productivity expectations, participating in community outreach and marketing initiatives to promote our services.

• Contribute to departmental goals by adhering to compliance standards and actively engaging in quality improvement initiatives.

• Ensure an excellent patient experience by demonstrating flexibility, accessibility, and responsiveness to patient needs.

• Support efforts to remove barriers to healthcare access and engage with notable patient outcome measurement initiatives.

Patient Population / Client Focus

Our practice focuses on a diverse patient demographic, ranging from post-operative rehabilitation to chronic pain management. As a physical therapist, you will work closely with patients suffering from various physical impairments, guiding them through personalized treatment modalities to regain independence and functionality. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.

• Current physical therapy license valid in Florida, with any additional certifications as required.

• Basic Life Support (BLS) certification from the American Heart Association is mandatory.

Preferred Experience

• A minimum of 2 years of clinical experience in a similar setting is preferred, showcasing a track record of excellent patient outcomes and engagement.

• Experience in outpatient rehabilitation facilities is an advantage, as well as familiarity with electronic health records (EHR) and therapy documentation systems.

• Strong interpersonal skills with the ability to build rapport with patients and work collaboratively with healthcare teams.

Compensation and Benefits
