Shift: Day | 7–10 Hour Shifts
Schedule: Full-Time; rotating weekends / on-call hours as assigned based on hospital volume

• Salary: $135,004–$200,000/yr, heavily based on years of experience

• Incentive Bonus: Up to 17.5%

• Sign-On Bonus: $10,000

Required Qualifications

• Bachelor's degree required (BSN preferred); Master's degree in Nursing, Health Administration, or Business Administration preferred

• Active Registered Nurse (RN) license, LCSW, or LMSW required (RN strongly preferred)

• 3+ years of acute care Hospital Case Management experience required

• 2+ years of Case Management leadership experience required

• Self-motivated, high performer with strong work ethics and executive presence

• Proven ability to review clinical/financial data and develop action plans independently

• Demonstrated track record of building strategic relationships with physicians, executive teams, and post-acute providers

• Strong conflict resolution, change management, and team-building capabilities

Responsibilities

• Direct and evaluate departmental operations, including the case management model, staffing (skill mix and FTEs), onboarding, and core competencies

• Provide operational oversight f or Case Management, Social Work, and the Chaplain Department

• Ensure strict adherence to care coordination and discharge planning processes, driving timeliness, documentation integrity, and quality care transitions

• Partner with the Facility CFO, Division CM Leader, CMO, CNO, and ancillary leaders to achieve facility throughput, clinical resource utilization, and financial goals

• Assess and improve performance by analyzing departmental data, evaluating operational workflows, and implementing sustainable quality improvement initiatives

• Ensure full departmental compliance with hospital policies, SOPs, Ethics & Compliance, and external regulatory agency standards

• Allocate resources effectively to staff the department and achieve productivity and quality targets

Additional Position Details

• Facility Size: 790 Beds (Level II Trauma Center)

• Reporting Structure: Reports directly to the VP of Case Management

• Department Size: ~110 FTEs (direct reports include 4 Managers, 1 Social Worker Supervisor, and an Administrative Assistant)

• Facility Volume: Over 34,000 annual admissions, nearly 88,000 ED visits, and 215,000+ outpatient visits

• On-Call/Coverage: Rotating weekend call/hours required depending on hospital volume
