Key Responsibilities\n

• Conduct thorough assessments of patients' mental health status, focusing on their emotional, behavioral, and social functioning to tailor interventions.

• Develop and implement personalized, evidence-based treatment plans that are culturally competent, addressing the individual needs of each patient.

• Provide direct psychotherapy services to patients experiencing mental health conditions such as anxiety, depression, and PTSD, focusing on coping mechanisms suitable for end-of-life care.

• Collaborate with an interdisciplinary team of healthcare professionals, including physicians and psychiatrists, to ensure a cohesive approach to patient care.

• Regularly monitor patient progress, adjusting treatment plans based on feedback and observed outcomes to optimize care strategies.

• Educate and support patients and their families by providing valuable information on mental health conditions, available treatment options, and community resources.

• Adhere strictly to ethical and professional standards, maintaining patient confidentiality and securing informed consent in all interactions.

• Utilize electronic health records (EHR) effectively to manage patient information and coordinate care seamlessly.

Patient Population / Client Focus\n

The LCSW will be primarily focused on working with patients in hospice care, including those facing terminal illnesses. This role requires sensitivity and an understanding of the psychological challenges that come with end-of-life scenarios. The LCSW will support not only patients but also their families, addressing grief, anxiety, and emotional distress stemming from the prognosis and associated life changes.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Valid and current state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
