Key Responsibilities\n

• Conduct thorough assessments of clients experiencing major mental illnesses and severe emotional disorders, both remotely and in the field.

• Develop personalized integrated care plans that connect patients with essential services and support networks.

• Provide evidence-based interventions, including brief therapy and crisis intervention, tailored to meet the individual needs of clients.

• Collaborate effectively within a multidisciplinary team to promote shared goals and enhance service delivery.

• Monitor client progress, ensuring adherence to treatment plans and facilitating transitions between various levels of care.

• Maintain records of client interactions, treatment plans, and assessments, ensuring compliance with organizational policies and regulatory standards.

• Provide on-call support as needed during and after regular program hours, addressing urgent client needs effectively.

Patient Population / Client Focus\n

Your primary focus will be on clients in a hospice setting. This includes individuals facing terminal illnesses and their families, all of whom may be dealing with significant psychological distress. Qualifications\n

• Master's degree in Social Work (MSW) with active licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The salary range for this position is USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
