Key Responsibilities\n

• Conduct comprehensive assessments to determine individual client needs and challenges, ensuring a deep understanding of their unique situations.

• Develop and implement personalized treatment plans based on evidence-based practices, including solution-focused psychotherapy techniques.

• Maintain accurate and confidential clinical records in compliance with regulatory standards to ensure the integrity of patient information.

• Work collaboratively with a dedicated treatment team, including psychiatrists, nurses, and therapist colleagues, to deliver holistic patient care.

• Educate and support clients and their families, providing resources and tools to help enhance their coping strategies and emotional resilience.

• Remain current with advancements in mental health research and practices to continuously refine your therapeutic techniques and interventions.

Patient Population / Client Focus\n

In this role, you will work primarily with adults facing a range of mental health challenges, including trauma, anxiety, depression, and grief, particularly within the hospice context. Our clients often experience significant life alterations and emotional struggles related to end-of-life issues, making trauma-informed care essential. You will be instrumental in providing both therapeutic support and compassionate guidance to improve patients' emotional well-being and enhance their quality of life during this critical time.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Valid Licensed Clinical Social Worker (LCSW) license in the state of Illinois.
