Key Responsibilities\n

• Develop and implement individualized support plans tailored to each client’s unique needs, focusing on overcoming personal challenges and dependencies.

• Conduct thorough assessments by interviewing clients to understand their current mental and emotional conditions, alongside their strengths and weaknesses.

• Address each case comprehensively, setting measurable and achievable goals to track the client’s progress effectively.
\n
• Continuously monitor and evaluate clients' development, adjusting treatment approaches as necessary to ensure optimal outcomes.

• Provide counseling and informed guidance during one-on-one sessions on the most suitable actions clients can take to enhance their emotional health.

• Maintain detailed records of client interactions and progress updates, reporting on their status to track the efficacy of implemented treatment strategies.

• Participate actively in case conferences and provide testimony in court when required, advocating for necessary resources for clients.
\n
• Serve as a key member of the multidisciplinary team, collaborating with medical professionals, therapists, and other social workers to coordinate comprehensive care.

• Facilitate connections to community resources and services that support client recovery and holistic well-being.

Patient Population / Client Focus\n

In your role as an LCSW, you will primarily work with clients facing significant emotional and psychological challenges, many of whom are navigating end-of-life issues or palliative care within the hospice setting. Your expertise will be essential in helping clients and their families to manage grief, anxiety, and other mental health concerns that arise during these sensitive times.

Qualifications\n

• A Bachelor’s Degree in Social Work, Psychology, or Sociology; a Master’s degree in Social Work (MSW) is preferred.

• Valid and active LCSW license to practice in Virginia.

• Demonstrated experience in a social work role, preferably within healthcare or hospice environments.

• Proficient understanding of social theories, practices, and ethical guidelines in social work.

• Strong interpersonal skills, with the ability to establish trust and rapport with diverse populations.

• Resilience in facing challenging situations, coupled with critical assessment skills.

• Willingness to undergo background checks and adhere to professional standards.

Preferred Experience\n

Experience working with end-of-life care and hospice populations is highly desirable. Familiarity with electronic health record (EHR) systems and experience maintaining patient confidentiality within an electronic framework will be beneficial. Strong documentation skills and experience in treatment planning and evaluation are also advantageous.

Compensation and Benefits\n
