Senior Social Worker – Perinatal Health UW Medical Center – Northwest Social Work offers an outstanding opportunity for a Senior Social Worker focused on perinatal health. The role involves partnering with patients, families, and the health care team to address medically related social and emotional needs, providing support, advocacy, and a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. Work Schedule Full‑time – 40 hours per week Day shift (Monday – Friday) with coverage of rotating holiday shifts Position Highlights Partner with patients, families, and the health care team to address medically related social and emotional needs. Provide a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. Coordinate care across the continuum, improving patient care through effective resource utilization and transition from acute to post‑acute settings. Standards of Practice Collaboration – Work collaboratively with patients, families, interdisciplinary team members, physicians, other practitioners, caregivers, and community resources. Communication – Provide timely, relevant, and accurate information to all parties involved in a patient’s care. Facilitation – Facilitate the progression of care by advancing the care plan to achieve desired outcomes. Coordination – Integrate the work of the health care team by coordinating resources and services necessary to accomplish agreed‑upon goals. Advocacy – Advocate on behalf of patients and caregivers for service access or creation and for the protection of patients’ health, safety, and rights. Resource Management – Ensure prudent utilization of all resources by evaluating options and balancing cost and quality. Accountability – Accept responsibility and accountability for achievement of optimal outcomes within scope of practice. Professionalism – Acquire and maintain knowledge and competence related to the expectations of the position and practice within scope. Primary Job Responsibilities Provide specialty consultation to other health care staff and teams regarding psychosocial aspects of patient care. Provide psychosocial support to patients and families. Complete clinical assessments as per department standards. Collaborate with the multidisciplinary team to develop and implement safe and timely discharge plans. Provide care coordination for patients with complicated social and medical histories. Provide case management and counseling services in specialized programs. Required Position Qualifications • A Master of Social Work degree from a program accredited by the Council on Social Work Education and one year of full‑time social work experience in a health care setting or equivalent (including practicum placements or employment experience).
