Location : White Plains, NY

Shift: Day Shift | Varied Hours (9A–5P)

Schedule: Per Diem | Weekend Availability Required

Pay Range: $60.63/hr – $76.96/hr

We are currently seeking an experienced RN Care Manager for a direct hire opportunity with a highly respected healthcare facility known for its exceptional workplace culture and patient care standards.

Key Responsibilities:

• • Perform comprehensive patient assessments upon admission
• • Coordinate multidisciplinary discharge planning and transitional care
• • Conduct Utilization Review (UR) activities and payer reviews
• • Collaborate with physicians, nurses, social workers, and care teams
• • Support readmission prevention and safe discharge planning
• • Educate patients and caregivers regarding aftercare and treatment plans
• • Utilize Milliman/InterQual criteria for medical necessity reviews
• • Ensure accurate documentation and compliance with department policies

Requirements:

• • BSN required
• • Active NYS RN License
• • PRI Certification (or willing to obtain within 3 months)
• • 3+ years of ED, Critical Care, or Med/Surg experience preferred
• • Previous Case Management experience preferred
• • Care Management certifications (CCM, CMC, etc.) are a plus
• • Strong communication, coordination, and critical thinking skills
• Ideal candidates will have strong clinical backgrounds, professionalism, stability in work history, and a passion for patient-centered care.
