The goal of the SSVF Housing Case Manager is to assist veterans in ending their housing crisis, enhance their independent living skills by providing supportive services and education, connect them with community resources, and empower them to maintain long‑term housing stability and self‑sufficiency. Qualifications Requires a Bachelor’s degree in social work or related field and less than five (5) years of experience in the field; at least five (5) years of experience in the field and no degree; or a Veteran with three (3) years of related work experience. Demonstrated ability to provide a reasonable combination of family needs assessment, resource access, advocacy, counseling, and cooperative work with local service providers. Excellent verbal and written communication skills, high degree of organization, and creative problem‑solving skills. Must have personal automobile, valid driver’s license, liability insurance, and be willing and able to travel between the counties served up to 70 % of the time. Must complete required case management and (Critical Time Intervention) CTI training within 90 days of hire. Belief in VOA’s core values—commitment, compassion, diversity, justice, and integrity. Responsibilities Build partnerships with community partners, service providers, and government agencies to address institutional and structural racism that limits housing opportunities for people of color experiencing homelessness. Work with Veteran families, including those of diverse cultural/ethnic backgrounds, with sensitivity to their housing needs. Actively remove barriers experienced by Veterans, especially people of color—economic mobility, affordable housing availability, criminal justice, behavioral health, and family stabilization—to provide equitable, secure housing. Explain, interpret, and apply for HUD‑VASH and Section 8 programs. Advocate for Veterans and act as liaison with landlords, rental agencies, community organizations, and other service agencies. Read, interpret, and explain rental contracts ensuring fairness and capability. Perform assessment and develop/monitor case plans; conduct necessary follow‑up activities. Establish linkages with appropriate agencies and service providers in the area/community. Provide referrals and coordinate with VAMCs and other providers. Assess Veteran needs and budget; develop individualized housing search plans. Create individualized housing plans and set goals with participants. Allocate resources based on participant need. Educate participants on supportive services availability and participant rights. Refer participants to financial planning, transportation, income support, fiduciary and representative payee, legal services, childcare, and housing counseling. Complete required documentation (including progress notes) within 48 hours of contact and enter data into the Homeless Management Information System (HMIS). Demonstrate good clinical judgment in decision making regarding participants. Relate to Veterans and their families in a culturally appropriate manner. Perform Performance Quality Improvement (PQI) duties as assigned by supervision and PQI committee. Work with other SSVF Case Managers, Intake Coordinators, and Outreach Workers; participate in regular case consultation. Develop cooperative arrangements with government agencies and community partners to provide housing for homeless families. Accompany Veterans to look for housing; assist in communication with landlords/property management. Conduct all housing inspections on eligible units. Conduct legal assessments and connect Veteran households with legal services to remove criminal justice barriers. Serve as resource for housing education and support tailored to Veteran needs. Collaborate with providers in ongoing reassessment of the Veteran household’s housing needs. Integrate Veteran needs/wants into their case plan. Conduct informational meetings and trainings with community partners, property management companies, and landlords on the SSVF program, housing needs, and fair housing guidelines. Negotiate rental rates, review leases, and provide feedback to the case consultation team. Mediate disputes between Veterans and landlords/property management. Investigate cases involving program and lease violations. Understand HUD, HUD‑VASH, Section 8, and other housing guidelines, rules, and regulations. Maintain active landlord and housing provider lists; establish new housing provider/partners. Serve as liaison between local HUD‑VASH teams, attend all case conferencing with HUD‑VASH and SSVF teams, and coordinate locally to meet Veteran needs. Understand barriers faced by clients in both urban and rural areas and serve them accordingly. Advocate with local public housing authorities and develop cooperative arrangements to provide housing for unhoused families. Conduct outreach to identify housing resources. Comply with all program policies and procedures and the Council on Accreditation.
