Job Description
Health by Design is actively seeking a fulltime perm family medicine/Internal Medicine Physician to start as soon as credentialed!


Perm Family Medicine
Coverage Location: Health by Design / Better Health Primary & Pediatric Care Center, Albuquerque, New Mexico

Dates of Coverage: ASAP, full-time, Monday - Friday from 8:00am - 5:00pm
Schedule: Monday - Friday, 8:00am - 5:00pm. No late clinics, no call and no after-hours charting.
Call Coverage: No
# of Providers Needed: One
Reason for Coverage: We are losing our full-time physcian, who has decided to go into private practice.
Practice Setting(s): [Clinic]
Leadership Position/Required to Teach: Would supervise 3 FNPs

Patient Population: Full-spectrum family medicine; babies to retirees
% Outpatient vs Inpatient: 100% Out patient
Average # of Patients Per Day: 8-14
Trauma Level Designation: N/A
Required Duties, Procedures, Skills, etc: Diagnose and treat acute and chronic medical conditions, Perform comprehensive physical examinations, Develop individualized treatment plans, Promote wellness and preventive care, Coordinate specialty referrals when appropriate, Order and interpret diagnostic testing, Participate in quality improvement initiatives, Supervise one or more NP team members.
EMR/Documentation: Elation
Additional Notes: We work in a value-based care model; outside the normal fee-for-service model. Ww are not taking/billing insurance and no RVUs.
