Responsibilities

• Provide personal care and support to residents as directed, assisting with activities of daily living.
• Aid with hygiene, grooming, dressing, mobility, and other daily tasks while preserving dignity and privacy.
• Monitor residents, document observations, and report changes to the supervising team; maintain accurate records.
• Support participation in daily routines and therapeutic activities; promote safety and adherence to care plans.
• Collaborate with nursing and behavioral health staff to implement care plans and coordinate with family or caregivers as appropriate.
• Foster a respectful, resident-centered environment and assist with de-escalation and safety as needed.

• Ability to work in a community mental health setting
