Facility:Dayton Children's - Main CampusDepartment:Clinical PharmacySchedule:Full timeHours:40Job Details:The Medication History Pharmacy Technician, with supervision of a pharmacist, assists in the process of obtainingmedication history for patients who will be admitted to the hospital. The focus of this technician is the accuratecollection of prescription information, allergy information, patient and/or family/guardian interview, anddocumentation of those activities. The complete collection of medication information may also include contactingpharmacies, long-term care facilities, and prescribers.Additionally, the Medication History Pharmacy Technician works under the supervision of the pharmacist followingestablished policy and procedures set forth by the hospital and pharmacy department. This role meets therequirements of being a qualified pharmacy technician as established by the State Board of Pharmacy and holds aCertified Pharmacy Technician license from a State Board of Pharmacy accredited agency. The technician adheres tosafety practices to ensure quality care for patients and maintains competencies outlined in pharmacy departmentpolicies.
