About the Role:
We are seeking a talented Physical Therapist (PT) to join our interdisciplinary rehabilitation team. As a Physical Therapist (PT) at Hope Creek Nursing and Rehabilitation Center, you will play a vital role in delivering high-quality care to patients with diverse needs and abilities. Your expertise will help patients achieve optimal physical function, wellness, and independence.
Responsibilities:
Assess and treat patients with various physical disabilities and conditions, including but not limited to stroke, orthopedic injuries, and chronic conditions
Develop and implement individualized treatment plans in accordance with patient goals and needs
Collaborate with healthcare team members to ensure comprehensive care and coordination
Conduct regular patient progress evaluations and adjust treatment plans as needed
Educate patients and their families on proper techniques, exercises, and self-care strategies
Document patient treatment plans, progress notes, and discharge summaries
Requirements:
Bachelor's or Master's degree in Physical Therapy from an accredited program
Physical Therapy licensure in the state of Illinois
Current CPR certification
Strong communication, organizational, and interpersonal skills
