• Develop and implement customized support plans for individuals based on their specific needs and goals.
• Conduct regular assessments to track progress and make necessary adjustments to support plans.
• Collaborate with a multidisciplinary team to ensure holistic care for each individual.
• Provide guidance and support to individuals in accessing community resources and services.
• Maintain accurate and up-to-date documentation of all interactions and progress notes.

• 3+ years of experience in customer service.
• Proficiency in Microsoft Office Suite.
• Strong verbal and written communication skills.
• Ability to work independently and in a team environment.
• Detail-oriented and highly organized.
