About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty. License: Must have license in hand. Certifications: CPR, BLS Must-Have: - Must be proficient in HomeCare HomeBase and OASIS and used it within the last 6 months. - Must have reliable transportation, a current driver's license, and agency-required liability insurance. - Must be open to traveling up to 75 miles from the assigned care center. - Must understand that all documentation must be completed within 24 hours of the appointment with the patient. - Must accept every patient on the schedule regardless of distance from previous appointments. - Must be comfortable with patient home visits and attending periodic meetings. Description: Provides coordinated and directed home nursing care according to physician orders. The care is based on established agency policies and procedures. The role involves the competent application of the nursing process. Responsible for supervising and evaluating home health aides and licensed practical nurses or licensed vocational nurses. Average patient visits per day range from 5 to 10. Documentation and travel activities hold different point values. No guaranteed hours are provided, and weekly productivity points must be met. Types of cases include start of care, wound care, and follow-up visits. The position may involve on-call requirements. Must be prepared for home visits and drive to patient homes daily. The position may lead to a possibility of hire after the contract assignment.
