DOC is seeking a highly motivated and qualified Registered Nurse for the Clallam Bay Corrections Center located in Clallam Bay, WA. Registered Nurses will receive shift differential in the amount of two dollars and fifty cents ($2.50) per hour for evening shift and night shift work. Employees may also qualify for one or both of these supplemental shift premiums in addition to the above. CBCC has an operating capacity of 858 male individuals located on the point of the Olympic Peninsula. Located in Clallam Bay, WA. It is 50 miles west of Port Angeles and looks out over the Strait of Juan de Fuca at Vancouver Island. PLEASE NOTE/TRAVEL INFO: We have limited Vanpool incentives available to DOC employees who are eligible for the Clallam Transit Vanpool program commuting to Clallam Bay Corrections Center or the Olympic Corrections Center. Please be sure to ask for vanpool availability and details on how to sign up! This position facilitates nursing components of Medication for opioid use disorder treatment (MOUD) and addiction medicine services in prisons to ensure eligible patients receive access to medications and discharge planning. This position is critical to support incarcerated individuals with maintaining or improving their wellness by bridging healthcare prison to community and facilitating access needed services during transition. The MOUD Nurse is part of a multidisciplinary team, working closely with Care Management and Health Services Reentry (particularly reentry nurses). As a response to an increase in fentanyl related deaths in the community, these positions will include educating patients and staff about medication assisted treatment. Research shows that access to MOUD care during transition saves lives and likely reduces recidivism. This work requires coordination of services with DOC Health Services, Prisons, Reentry and Community Corrections Staff. In collaboration with the facility Care Management Team and Health Services reentry team, job duties focus on the Department's Strategic Goal of Continuity of Care Planning for Health Services patients. This supports the mission of the Health Services division by providing necessary, comprehensive, quality health services. Duties
Coordinate with the Re-Entry Navigator to ensure MOUD services are part of Continuity of Care planning. Provide education to patients on general substance use disorder basics as well as available treatment options, including currently available medication options within DOC system and how to access these treatments. Provide education to patients who are starting medications for treatment of opioid use disorder with expected side effects, and follow up with patients at regular intervals during medication induction to assess side effects and progress. Facilitate lab tests, medical appointments, follow up services needed and release medications upon release Educate about use of Narcan for opioid overdose reversal. Coordinate care for release plan for patients on MOUD with the Reentry Care Navigator team. Work in collaboration with Addiction Medicine Director in ongoing improvement of process of MOUD treatment within DOC as well as improvement of the reentry and handoff processes. Collaborate with MOUD nurse team to provide ongoing support to nursing staff at additional facilities that do not currently have an MOUD nurse; this can include phone consultation, patient education via virtual visits or traveling to additional facilities to provide consultation to staff and/or patients on area of expertise. Adhere to all DOC and Department of Health policies and procedures pertaining to medication administration, including controlled substances, procurement of medications, and reporting to the supervisor medication errors or potential sources for mistakes or mishaps. Maintain current, complete and confidential patient records in accordance with Health Service Policies and Procedures. Assess the subjective and objective symptoms of patient's medical and emotional conditions during nursing contact; initiate appropriate intervention according to Health Services policies and procedures, treatment plans, and protocols. Assess, monitor, and interpret objective and subjective data collected about the health status, clinical condition or situation by interview, observation, inspection, and examination to identify patterns and variances from the desired outcome. Develop nursing diagnoses and outcomes to be achieved as a result of nursing intervention. Plan the delivery of health care, defining timeframes and pathways for delivery of services, prioritizing and sequencing care to be provided, ensuring continuity of care over time and place. Implement care plans by providing direct patient care, directing others about the urgency, timeframes, and methods to deliver health care on and off site, determining when needs for care or services become more urgent, require clarification, or require another type or level of intervention. Take action as necessary to obtain provider orders and accomplish prescribed interventions in a timely and clinically responsive manner. Analyze and evaluate the structure, process, and timeframes for service delivery, the expected effects of treatment or intervention, and the outcomes to be achieved. Revise the plan of care as necessary to prevent complications, and intervene to address new or emerging problems or to improve care and service delivery. Initiate and support discharge planning and coordination of information and resources for patients referred to caseload in partnership with facility Reentry Nurse. Partner closely with facility reentry nurse to provide healthcare transition planning to patients, with a focus on comprehensive transition planning for MOUD patients. Conduct patient interviews to determine specific medical, mental health, substance use disorder and other transition needs. Create/ contribute to a personalized Health Services Reentry plan based upon patient interview, multidisciplinary team meetings and records review. Elicit participation of the team in developing, assessing, and revising individual patient Continuity of Care plans. This may include contacting institution Health Services staff, Program Services staff, Security staff, Transport staff, Administration staff, and when appropriate, the patient family Initiate and coordinate Community referrals to appropriate service providers and agencies such as Disability Services, Social Security, Dialysis services, VA services, Community Care Facilities, Personal Care providers, and Community Corrections transition services staff. Initiate and coordinate Medical Facility placement for specialized care after release. Work with Community resources and providers to facilitate eligibility and placement process and to locate appropriate care facility. Work with Care Facility staff to ensure appropriate continuity of medical care. (Plan with Community Corrections and DOC Administration to assure any necessary County waivers or possible adjustments in parole plans due to medical needs.) Assess durable medical equipment needs and provide equipment necessary for immediate support upon release, such as wheelchairs, walkers, canes, or glucometers. Provide assistance to obtain necessary medical transition information necessary for continuity of care. Reentry Nurse and MOUD nurse provide cross coverage to ensure MOUD and Reentry needs of patients are met.
Qualifications
Registered Nurse 2 Required Qualifications: Initial and continuous unencumbered licensure in the State of Washington as a Registered Nurse. OR An unencumbered Multi State License from a participating Compact Licensure State as outlined by the Nurse Licensure Compact and Department of Health with completion of Suicide Prevention, EDIR training, and MLS demographic survey. One (1) year experience working as a Registered Nurse Training by the American Heart Association in Healthcare Provider CPR. Proficient using computer-based programs to schedule, monitor and track patient information, perform data entry and word processing, and communicate via email, as well as other programs that may be needed to complete assigned work. Ability to work independently and with limited supervision. In addition to the above, this position requires: Analysis - Uses data and information in a clear and rationale thought process to assess and understand issues, evaluate options, form accurate conclusions and make decisions. Identifies key facts, understands the nature and relationship between data elements, recognizes underlying principles or patterns in an array of data, and determines if additional data should be collected. Uses judgment and critical thinking in the absence of clear guidelines to draw conclusions about further action. Observation and Inspection - Critically observes and inspects people, conditions, and the environment to detect problems. Acutely aware of critical details, alert and attentive when performing other work, understands normal conditions and constantly compares the current status to the norm to identify subtle changes and incremental deviation, makes informed judgment about when a change in current status must occur and determines appropriate action. Uses insight to identify contributing and mediating factors. Understands thorough documentation and other forms of communication to cause others to understand and take appropriate action. Desired Qualifications: Experience working as a Registered Nurse in an acute care setting or a correctional facility. Certification as a Correctional Healthcare Professional. Experience as a Recovery Center, Detox Center, or public health facility treating drug addiction Current Cerification or willingness to achieve certification in Addictions Nursing (CARN) Experience as a nurse case manager for at least 1 year, or current certification, OR education Bachelor's of Science
