Clinical Pharmacist - West Columbia

Job Description Option Care Health, Inc. is the largest independent home and alternate site infusion services provider in the United States, with over 8,000 team members including 5,000 clinicians. Responsibilities Determine the suitability of patients for home care, collecting and organizing all patient-specific information needed to assess suitability according to Option Care policies. Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans, making recommendations for modifications in prescribed medication therapy or monitoring parameters. Modify pharmacy care plans based on evaluation of monitoring data and patient information, using pharmacokinetic principles and assessing laboratory values. Provide medication‑use education to staff, professionals, patients, and caregivers using effective counseling techniques. Ensure continuity of pharmaceutical care, communicating pertinent information between home and other patient‑care settings, and preparing and dispensing medications following policies, procedures, and regulations. Maintain and troubleshoot medication administration equipment, complete quality assurance reports, and monitor inventory systems for controlled substances. Respond promptly to drug information requests from healthcare providers and patients, formulating responses based on literature analysis. Manage investigational drug products according to regulations and policies, and optimize the use of pharmacy support personnel. Comply with accreditation, legal, regulatory, and safety requirements and maintain confidentiality of patient and proprietary information. Basic Education and Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license issued by the State Board of Pharmacy. At least one year of experience in sterile IV admixture procedures, or knowledge of non‑sterile compounding, immunizations and medication therapy management, or completion of an ASHP‑accredited Pharmacy Residency Program, Advanced Pharmacy Practice Experience, or equivalent approved experience. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction. Proficiency in Microsoft Word, Excel, and PowerPoint (basic level skills as described). Travel Requirements Willing to travel up to 5% of the time for business purposes within the state. Preferred Qualifications & Interests Completion of an ASHP‑accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience. Experience in home care, hospital, or other clean‑room setting. Management Experience At least one year of indirect management experience, including assistance in training and assigning work to other team members. Salary Pay range: $92,004.26 – $153,332.93, determined by education, experience, knowledge, skills, and internal equity.
