Join a team at the forefront of value-based care and discover the meaning behind.The Nurse Intern - Field Based Health Coordinator will be the primary care manager for a panel of members with complex medical/behavioral needs.Care coordination activities will focus on supporting members medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.The Nurse Summer Internship is full-time, 40 hours per week for 10 week starting June 2, 2026. This position does travel to visit community centers, facilities, community activities, and patient homes across a designated territory. Mileage reimbursement will be provided. This role is hybrid as the intern will work from home when not in the field.The Nursing Summer Internship Program is designed to provide nursing students the opportunity to gain hands on experience in areas within the nursing field outside of hospital bed-side patient care. During the experience you will get exposure to patient care needs through the healthcare continuum, enrichment opportunities such as A Day in the Life of a Nurse Series, Enterprise Foundations, networking, and student cohort connections.Primary Responsibilities:As a Nurse Intern for Community & State - Hawaii Health Plan, you will learn how to manage the care of members in their homes or other community settings by completing in-person health assessments and coordinating patient care, focusing on disease management, and keeping members healthy and independent. Under the supervision of our RNs and providers, you will learn how to:Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needsDevelop and implement person centered care plans to address needs including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management and member safety in alignment with evidence-based guidelinesPartner and collaborate with internal care team, providers, and community resources/partners to implement care planProvide education and coaching to support member self-management of care needs and lifestyle changes to promote healthSupport proactive discharge planning and manage/coordinate Care Transition following ER visit, inpatient or Skilled Nursing Facility (SNF) admissionAdvocate for members and families as needed to ensure the members needs and choices are fully represented and supported by the health care teamAttend community fairs and outreach events held during the week or weekendYoull be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Currently enrolled in, or recently graduated from, an accredited RN Bachelors Degree program, with graduation expected within 12 months of the internship start date (before or after)Current Basic Life Support (BLS) certification1+ years of Clinical ExperienceExperience with MS Office, including Word, Excel, and OutlookAvailable to work full-time, 40 hours per week during the duration of the summer internship program (from June 2 - August 7)Legally authorized to work in the US without any restrictions. Proof will be requiredMust be eligible to work in the U.S. without company sponsorship, now or in the future, for employment-based work authorization (F-1 students with practical training and candidates requiring H-1Bs, TNs, etc. will not be considered)Access to reliable transportation that will enable you to travel to community centers, facilities, community activities, and patient homes across a designated territory.Preferred Qualifications:3.0 or higher cumulative GPA Knowledge of clinical terminology Demonstrated oral and written communication skillsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
