Our welcoming environment fosters collaboration among staff and promotes an engaging experience for our patients.Key ResponsibilitiesConduct comprehensive evaluations of patients' physical abilities and limitations within a home care setting, specifically designed for transitional support as they recover from injuries, surgeries, or illnesses.Design and implement individualized treatment plans that incorporate a variety of therapeutic modalities such as therapeutic exercise, manual therapy, and patient education to alleviate pain and enhance mobility.Continuously assess patient progress and make necessary adjustments to treatment plans, ensuring that each therapeutic goal is achievable and aligned with the patient's recovery path.Document all patient interactions, treatment responses, and outcomes accurately in compliance with legal and organizational requirements, using our electronic medical record (EMR) system.Educate patients, caregivers, and family members on effective self-care practices and the proper use of assistive devices, ensuring understanding and compliance for optimal recovery.Participate in regular professional development through workshops and continued education opportunities to stay abreast of industry advancements and improve therapeutic techniques.Patient Population / Client FocusOur facility serves a diverse patient population, including children and adults with various diagnoses. You will work with individuals recovering from orthopedic injuries, neurological conditions, post-surgical rehabilitation, and chronic pain conditions. Your role will be pivotal in helping patients regain their independence and functional abilities, thereby improving their overall quality of life.QualificationsA Doctorate degree in Physical Therapy (DPT) is required.A valid physical therapy license in the state of Florida.Strong verbal and written communication skills to effectively interact with patients, families, and team members.Proficiency in computer skills, particularly in word processing and using electronic medical records.A deep commitment to patient care and a passion for helping others achieve their health goals.Preferred ExperienceWhile a minimum of 2 years of experience in a clinical setting is preferred, we encourage new graduates with a strong clinical rotation in relevant areas to apply.Experience with assistive devices and adaptive therapy techniques is a plus.Demonstrated ability to work collaboratively within a multidisciplinary team environment.Compensation and BenefitsOur competitive salary range of $100,000 – $120,000 annually reflects our commitment to attracting the best talent.
