We are currently seeking an experiencedLVN Home Health Clinical Coordinatorto join our leadership team and support the ongoing success of our clinical operations.
Position Overview:
The LVN Home Health Clinical Coordinator oversees the coordination and supervision of daily clinical activities to ensure timely, high-quality care. This position works closely with the Director of Nursing, field clinicians, and administrative staff to uphold regulatory compliance, improve clinical outcomes, and enhance the patient experience.
Responsibilities:
- Coordinate and oversee patient care delivery, ensuring compliance with physician orders, agency protocols, and regulatory standards
- Manage and monitor staff assignments, patient schedules, and case load distribution
- Review and approve clinical documentation
- Facilitate case conferences, patient status reviews, and interdisciplinary collaboration
- Provide clinical guidance, training, and support to field staff
- Assist with admissions, recertifications, and discharges as needed
- Ensure timely submission of documentation to support billing and regulatory deadlines
- Participate in quality assurance, performance improvement (QAPI), and survey readiness
- Maintain up-to-date knowledge of CMS Conditions of Participation and home health regulations
Qualifications:
- Active LVN license in the state of Texas (required)
- Minimum 2 years of clinical experience in home health (required)
- Prior supervisory or case coordination experience (preferred)
- Proficiency with EMR systems (Kinnser/WellSky preferred)
- Strong understanding of Medicare regulations
- Excellent leadership, communication, and problem-solving skills
- Ability to prioritize and multitask in a dynamic environment
