POSITION DESCRIPTION

1. TITLE OF POSITION: Community Health Worker

1. NATURE OF POSITION: This is a safety-sensitive position. The Community Health Worker (CHW) is a key member of the interdisciplinary care team, supporting patients and families in navigating healthcare services, accessing community resources, and improving health outcomes. The CHW promotes patient understanding, removes barriers to care, and provides education, advocacy, and ongoing support to enhance overall well-being
2. QUALIFICATIONS:
   1. Minimum one (1) year of medical experience preferred
   2. Community Health Worker training preferred
   3. Completion of GSP Health Clinical Assistant (CA) orientation and 30 days in CA role required
   4. Basic Life Support (BLS) certification required
   5. Strong communication, organizational, and problem-solving skills
   6. Ability to work with diverse populations with empathy and professionalism
   7. Proficient in Microsoft Office and basic computer skills
   8. Knowledge of community resources and ability to build connections
   9. Self-directed, flexible, and team-oriented
3. SALARY GRADE AND CLASSIFICATIONS: Non-Exempt

1. RESPONSIBLE TO: Case Management Lead

1. SUPPORTIVE/CHARGE RESPONSIBILITIES:
   1. Assist patients and families in navigating healthcare services and community resources
   2. Support care coordination through communication with providers, case managers, and care teams
   3. Conduct Medicare Annual Wellness Visits and Health Risk Assessments
   4. Educate patients on conditions, medications, and preventive care
   5. Identify and address barriers to care, including financial, transportation, and social needs
   6. Assist with applications for assistance programs and service
   7. Provide advocacy and support for patient decision-making
   8. Track referrals and follow up to ensure access to care
   9. Conduct follow-ups via phone, clinic, home, or community settings
   10. Support goal setting, motivational interviewing, and patient engagement
   11. Maintain accurate documentation and reporting
   12. Promote cost-effective, quality care outcomes
   13. Serve as liaison between patients, families, and community agencies
   14. Stay informed of available community resources and services
   15. Due to grant funding, this position will be required to attend a virtual RHTP Program Goals & Protocols Orientation, complete virtual OSU Foundational CHW Curriculum, attend one day in person at the OKPCA Conference CHW Session, shadow CHW staff at a partner FQHC, shadow a CHW at a health department, and attend other meetings and trainings per the RHTP CHW Grant requirements.
