This role requires collaboration with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, ensuring a holistic approach to patient care.Key ResponsibilitiesThe Physical Therapist will be tasked with the following:Designing, implementing, and documenting individualized physical therapy treatment plans based on each client's specific needs and physician's directives.Assessing clients' functional abilities using diagnostic methods to identify strengths and areas requiring rehabilitation.Overseeing the implementation of treatment plans by physical therapy assistants, ensuring adherence to best practices and compliance with state regulations.Providing therapeutic exercises, mobility training, and educational resources to clients and their families, optimizing their engagement and cooperation.Evaluating home environments and making recommendations for adaptive modifications that will enhance clients' independent living capabilities.Monitoring client progress and reporting outcomes to the interdisciplinary team, facilitating adjustments to treatment plans as necessary.Maintaining clear and concise clinical records, including evaluations, care plans, and discharge summaries, in compliance with organizational and regulatory standards.Participating in case conferences to collaboratively review treatment effectiveness and strategize comprehensive care approaches.Providing ongoing education and support to clients and family members regarding rehabilitation goals and the use of therapeutic devices.Remaining accessible to assist physical therapy assistants during treatment sessions.Patient Population / Client FocusThis role focuses on a diverse patient population requiring physical rehabilitation within their homes, including elderly clients, those with chronic diseases, and individuals recovering from surgeries or injuries. The physical therapist will address various issues, from pain management to mobility enhancement, crafting treatment plans aimed at maximizing each client's functionality and quality of life.QualificationsThe ideal candidate will possess:A Doctorate degree in Physical Therapy (DPT) and a valid state licensure for practice in New Jersey.Current CPR certification and a valid driver's license with automobile insurance.Strong interpersonal skills to effectively communicate and engage with clients, families, and multidisciplinary team members.A commitment to delivering high-quality patient care with a positive and team-oriented attitude.Preferred ExperiencePreferred qualifications include at least two years of experience as a Physical Therapist, particularly in home care or acute settings.
