Pride Health is hiring a Case Manager to support our client's healthcare facility in Brooklyn, NY.

Job Details: 
Schedule: Sunday – Thursday, 11:00am – 7:00pm
Location: Brooklyn, NY 11207
Job Type: Local contract
Contract Length: 13 weeks with potential extension  
Pay Range: $25.00 to $28.00/hour*

*Pay offered is based on experience, expertise, credentialing, and education.

Job Duties: 

• Support in managing a program census of up to 21 clients, provide support and resources around individualized service plan goals, and maintain timely and accurate documentation.
• Connect clients with community resources, including medical, mental health, substance use, vocational, and basic needs services.
• Provide crisis intervention, problem-solving support, and client advocacy.
• Conduct intake assessments, discharge planning, and follow-up care (7-day and 30-day check-ins).
• Coordinate with community partners and internal teams to ensure quality care and successful outcomes.
• Maintain accurate documentation in compliance with agency standards.
• Participate in team meetings, supervision, training, and community-based activities that support client engagement and integration.

Education Requirements: 

• Education: Bachelor’s degree in human services or related field required
• Master’s degree in human services or related field preferred
• Understanding of a broad range of mental health conditions and client needs such as mental illness, substance abuse
  issues, Crisis /Residential/Respite 24 hour centers, and/or a history of
  homelessness required.
• Strong communication, case management, care coordination, and organizational skills.
• The ideal candidate will be creative, innovative, open to change and willing to
  contribute new ideas
• Skills/Specialties/Technology: Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
