Summary The Child Center of NY, in coordination with NYSPI, began an OnTrack program through OMH to work with teens and young adults that are experiencing First Episode Psychosis. The Assistant Director in this role will be a part of an interdisciplinary team that works closely with the New York State Psychiatric Institute and OMH to provide an evidence-based treatment model to a caseload of 50 clients concurrently. The Assistant Director will also act as a primary clinician on the team by providing direct care to clients using a variety of social work modalities, including individual and group therapy, in order to help them maximize their level of functioning. The Licensed Master Social Worker will act as the Primary Clinician on our OnTrack NY Team. The Primary Clinician is the primary therapeutic resources for participants and their families. As the team member who is most frequently the primary contact for participants and their families, the Primary Clinician is responsible for ongoing engagement, assessment, and relationship building with participants and their families. Essential Functions: Serve as the point person for the participant and their family; Provide services in the community (off-site) as needed based on participant and family preferences. Conduct needs assessments; Engage in safety planning and wellness management planning with participants; Connect participants and families with the services and supports they need; Provide behavioral interventions and take an especially active role in the family support domain. Conduct clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education. Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program; Provide supportive therapy; Develop and modify treatment using a shared decision-making process with clients Work with participants and family for transition from the team to other services and supports, and ensure stable links at discharge. Minimum Qualifications: Master's Degree in social work, mental health counseling, or similar master's degree Experience in community-based mental health services 3-5 years of direct care experiences with adolescents and/or young adults with serious mental illness a plus Qualifications Preferre d: Bilingual in either Spanish or Mandarin Physical demands and work environment: The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
