Key Responsibilities

• Deliver high-quality, compassionate patient care in both the outpatient and acute care settings, ensuring that every patient receives individualized attention and support.

• Implement and customize treatment protocols for assigned patients, adhering to established plans developed collaboratively with other healthcare team members.

• Conduct thorough evaluations, including comprehensive history reviews and specialized assessments, to determine the most effective treatment strategies for each patient.

• Prepare detailed documentation with accuracy and timeliness, capturing essential information regarding patient care activities, progress notes, and discharge summaries.

• Lead discharge planning efforts and ensure patients understand recommended follow-up care, maintaining clear communication with all stakeholders involved in the patient's recovery.

• Facilitate open, respectful communication with colleagues, medical personnel, patients, and their families to enhance the overall care experience.

Patient Population / Client Focus

Qualifications
