Clinical Pharmacist

Overview A Clinical Pharmacist contributes to AHF by expanding primary care functions of pharmacists, including health promotion and disease prevention, vaccine administration, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs, and other subject matters, in collaboration with other AHF providers. The Clinical Pharmacist enhances the reputation of AHF by contributing to best practices and ensuring consistent application of processes, procedures and policy throughout the organization. The organization maintains a collaborative structure where staff are accountable to multiple leaders. Role focus : The Clinical Pharmacist will work as part of a close-knit team to deliver individualized health promotion and disease prevention programs, including administration of immunizations, diabetes management, and smoking-cessation programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority. Responsibilities Provide individualized health promotion and disease prevention services in collaboration with AHF providers. Administer vaccines and immunizations as part of preventive care. Participate in long-acting HIV treatments and diabetes monitoring programs. Support nicotine reduction programs and other SME programs developed by AHF. Apply best practices and ensure consistent execution of processes, procedures, and policies across locations. Collaborate with the organization to maintain alignment with AHF core values and mission.
