TRAVEL LICENSED PRACTICAL NURSE (LPN) | CONTRACT ASSIGNMENT JOB OVERVIEW

Seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a contract opportunity in a long-term care and rehabilitation setting. The LPN will provide direct nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary healthcare team to deliver exceptional patient outcomes.

SHIFT DETAILS

• Shift Time: 6:00 PM 6:30 AM
• Meal Break: 30 Minutes
• 12.5-Hour Night Shift
• Schedule: Wednesday, Thursday, and Friday

GUARANTEED HOURS & PAY

• Guaranteed Hours: 36 Hours Per Week
• Pay Rate: $33.48 - $39.06 per hour

REQUIREMENTS

• Active Licensed Practical Nurse (LPN) License in good standing
• Current BLS certification preferred
• Previous long-term care, skilled nursing, rehabilitation, or related healthcare experience preferred
• Strong clinical assessment and documentation skills
• Excellent communication and interpersonal abilities
• Ability to work effectively within an interdisciplinary healthcare team

KEY RESPONSIBILITIES

• Provide direct nursing care and treatments to residents
• Administer medications and monitor resident responses
• Assess resident conditions and promptly communicate changes to the healthcare team
• Maintain accurate and timely clinical documentation
• Assist with individualized care planning and implementation
• Collaborate with physicians, nurses, therapists, and support staff
• Educate residents and families regarding care plans and treatment goals
• Promote resident safety, dignity, and quality of life
• Ensure compliance with facility policies, procedures, and regulatory standards

• Guaranteed 36 hours per week
• Competitive pay rate of $33.48 - $39.06 per hour
