Seven Healthcare are seeking an experienced RN for a travel Case Management role specializing in wound care. This position involves working within various counties in Wisconsin, performing home visits and collaborating with care teams to provide comprehensive patient care. Candidates will work in home and healthcare settings, aiding patients across Fond du Lac, Dodge, Columbia, and Winnebago counties.

- Wound Care Experience
- Certified CPR/BLS
- Valid Driver's License
- Reliable Vehicle with Valid Car Insurance
- Associate's or Bachelor's Degree in Nursing or related field
- Experience with Electronic Medical Records; Home Care Home Base highly desired
- Case Management certification
