RN: Case Manager

RN: Case ManagerOur client, a healthcare company, is looking for an RN: Case Manager for their Fond du Lac, WI location.Responsibilities:Perform comprehensive assessment of high risk members.Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member.Address, identify, and continuously reassess cost-efficient, appropriate levels of care.Put non medical support in place to ensure compliance with treatment plans (such as housing and transportation).Coordinate care transitions as needed.Coordinate with physicians, social workers, discharge planners, and claims professionals in transitioning patient to appropriate level of care.Engage specialty resources as needed to achieve optimal resolution.Keep detailed records of clinical, functional, and fiscal outcomes during the management process.Identify opportunities for health promotion and illness prevention.Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes.Act as patient advocate by protecting privacy and confidentiality issues.Requirements:RN License for WIWound Care ExperienceCertified CPR/BLSValid DL Reliable Vehicle Valid Car InsuranceHome Care Home Base-HCHB Highly Desired if they don't have this experience Matrix Care is acceptable or any Electronic Medical RecordFor a candidate with no Hospice experience it would vary based on experience with: Working with frail elderly Working with patients in a Home setting Working with /in Skilled Nursing (Nursing Homes) Assisted Living FacilitiesAs well as any of the following preferred experience: Case Management - oversight/collaboration with other members of the Hospice team.( CNA, Social Worker, Chaplain, Music Therapist) Care Planning Documentation - narrative assessment of visit/patient status Coordination of care with Families, Facility staff and families Knowledge of End of Life Medications Comfort in education of patient, family and staff of end of life signs and symptoms, disease processes and symptom management.Wound CareIDG Documentation/ Recertification Pain ManagementSkills competency checklist-required and must be uploaded during compliance checksElder Abuse trainingHIPAA trainingInfection prevention and controlOSHA complianceQualifications: Associates or Bachelors Degree in Nursing or related field.Ability to analyze complex medical information and make rational decisions.Case Management certification.Why Should You Apply?Health BenefitsReferral ProgramExcellent growth and advancement opportunities
