Board Certified or Board Eligible (0-5 years of residency) in OB/GYN - REQUIRED
CO license (or willing to obtain) - REQUIRED
Clean malpractice - HIGHLY PREFERRED
Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
Does the provider require visa support - J1 or H1B? (note in 'Additional Submission Details' field) - REQUIRED
DEA (must be obtained prior to start) - (note in 'Additional Submission Details' field) - REQUIRED no exemptions
