Case Manager

Job Title: Case Manager – Manhattan, Brooklyn and Bronx Educational Requirements: Bachelor’s Degree or high school diploma/GED with relevant experience. Qualifications and Experience: Experience working with mental health, crisis intervention and managing trauma triggered behavior. Excellent time management skills; stress resistant, flexible; team player; open to feedback; creative; excellent organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone. Preferred knowledge of AWARDS. Pay/Salary: 40,000-44,000 Annually Scope of Position: Under the supervision of the Program Director/Coordinator the Case Managers will be responsible for providing supportive services to formerly homeless adults diagnosed with serious mental illness, seniors, veteran’s, medically frail, and individuals with a forensic history who are living in apartments. The Case Manager will hold individual meetings as well as co‑facilitate groups while completing timely documentation in accordance with agency policy. Case Manager Job Responsibilities: Deliver comprehensive case management services, including housing assistance, service referrals, benefits maintenance, client escorting, assessments, and coordination with external providers. These services are primarily provided in the client’s apartment and in community settings. Work with the clinical High‑Risk Coordinator to provide case management and interventions. Assist in identifying strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system. Use Motivational Interviewing techniques to help clients develop skills needed to maintain housing and develop natural supports. Timely completion of case notes and other required documentation for every client contact as per agency policy. Assist with Medication Management and Front Desk Monitoring. Co‑facilitate groups if requested, assist in preparing the program for audits. Provide crisis intervention, as necessary, utilizing de‑escalation techniques. Compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation, and incident reporting. Ability to work with consumers/residents, families, community tenants, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences. Conduct apartment inspections on multiple floors, utilizing stairs. Provide escorts to clients throughout the community using public transportation. Immediately report serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. Complete incident reports in accordance with Bridge policy. Account for consumers, perform diligent searches, and file missing person reports on consumers not accounted for in accordance with policy and procedure. Attend staff meetings, supervision, and trainings as required. Any other duties as may be assigned, including the potential of supporting other programs within the agency portfolio.
