Care Coordinator - Home Care Services
Location: Columbus, Georgia | Full-Time

We're looking for a Care Coordinator who is proactive, empathetic, and thrives in a people-centered environment.
- Serve as the primary point of contact for new client referrals and care transitions
- Coordinate personalized in-home care plans with compassion and attention to detail
- Ensure continuity and consistency of care by matching clients with the right caregivers
- Monitor client satisfaction and well-being through regular check-ins and visits
- Support families with guidance, resources, and care planning insight
- Supervise and support caregivers, including performance evaluations and ongoing communication
- Collaborate with healthcare professionals, insurers, and internal teams to streamline services
- Participate in rotating on-call responsibilities to maintain service continuity
- Education : High school diploma or GED required; college degree or relevant certification preferred
- Experience :
- At least 2 years in a healthcare or home care setting
- Experience in care coordination or client services strongly preferred
- Licensure (must have one of the following valid in Georgia):
- Licensed Practical Nurse (LPN)
- Certified Nurse Aide (CNA)
- Personal Care Assistant (PCA)
- Solid understanding of elder care, chronic illness, disability support, and home-based safety
- Strong communication and relationship-building skills
- Competency in meal support, hygiene assistance, infection control, and home safety procedures
- Ability to document services accurately and follow care instructions
- Must meet state-required training standards
- Must pass background, reference, and driving record checks
- Valid driver's license, reliable vehicle, and current auto insurance
