Key Responsibilities

• Evaluate physical therapy needs of assigned patients and establish individualized goals and plans of care
• Assess, plan, treat, evaluate, and implement a Plan of Care (POC) for all clients seen
• Provide age-appropriate care to patients ranging from pediatric to geriatric populations
• Complete documentation of all patient care activities in a timely manner within the electronic medical record
• Communicate and interact with patients, families, physicians, and staff in a manner consistent with organizational core values
• Demonstrate understanding of admission processes, treatment coding, and appropriate billing practices

Required Qualifications

• Minimum 1 year of Home Health physical therapy experience required
• Active Illinois Physical Therapist license (must be active at time of submission)
• BLS certification required (must be active at time of submission)
• Valid driver's license and current auto insurance required
• COVID-19 vaccination required — declinations will not be approved
• Local candidates are welcome; candidates located outside the service territory may not be considered

Preferred Experience / Skills

• OASIS experience strongly preferred
• Start of care (SOC) experience strongly preferred
• Experience working with multidisciplinary healthcare teams in a home health setting
