Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate Formulates a plan of care for individual needs, including patient population needs. Participates in the planning of routine transitional health care needs (i.e. treatment options, patient placement options) Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care Ensures efficiency in the delivery of care to determine immediate priorities and to offer solutions for problems Makes independent clinical decisions based on knowledge drawn from education and experience Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict Job Requirements Minimum Qualifications: Minimum of one (1) year of experience as a Physician Assistant in an outpatient primary care clinic, community health center, etc. Must hold an academic degree and completion of a specialized Physician Assistant Training Program Successful completion of a national physician assistant examination prior to being licensed by the Physician Assistant Board in California. Licensed as a Physician Assistant in good standing from the California State Physician Assistant Board. N National Provider Identifier (NPI) number satisfactory report from the National Practitioner Data Bank Ability to successfully meet Dignity Health's credentialing and privileging processes upon hire and biannually thereafter BLS certification PA license Preferred Qualifications: Ability to verbally communicate in Spanish is highly desirable.
