Elevate Healthcare is seeking a dependable Care Coordinator to serve in a specialized Float capacity across multiple care teams.
The Float Coordinator plays a defined operational role focused on workflow continuity, documentation accuracy, and structured execution across programs. This position supports existing teams under the direction of assigned team leads.
This is a specialized execution role. Success is defined by reliability, adaptability, and consistent performance within established systems.
This position is designed as an ongoing specialized role within the organization.
Essential Functions
Execute established workflows accurately within assigned care programs
Maintain timely and accurate documentation across EMR platforms
Perform follow-up communication according to defined protocols
Track, route, and monitor tasks to completion
Provide structured coverage across programs during high-volume periods
Integrate into different team processes while maintaining workflow consistency
Escalate concerns according to established policy and leadership structure
Role Structure
This position rotates between programs based on operational needs. The Float Coordinator does not maintain primary ownership of a single program but instead ensures seamless workflow execution across teams.
Authority for workflow design, policy decisions, and program direction remains with program leadership. The Float role is responsible for implementing established processes with precision and professionalism.
Performance is measured by accuracy, dependability, and the ability to transition between assignments without disruption.

Qualifications
Prior healthcare administrative or clinical support experience preferred
Experience navigating EMR systems and digital platforms
Demonstrated reliability in structured work environments
Work Environment
Hybrid position combining in-office collaboration with remote flexibility based on performance and operational needs. This role may rotate between programs as coverage requires.
Compensation
Starting at $17/hour DOE
