Community-Based Case Manager: Empower & Connect

AltaPointe Health

AltaPointe Health in Kentucky is seeking a Case Manager to support consumers across mental and physical health, living, learning, and social environments. You will travel to consumers' settings, assess needs, set goals in ISP, and link to community resources to promote recovery and independence. Responsibilities include ongoing monitoring, crisis de-escalation, collaboration with treatment teams, and thorough documentation per AltaPointe standards. #J-18808-Ljbffr AltaPointe Health
