- PART TIME PHYSICAL THERAPIST- 18-24 HOURS PER WEEK SIGN ON BONUS
- PER DIEM PHYSICAL THERAPIST- WEEKDAYS and/or WEEKENDS

- Assumes responsibility and accountability for the care of those residents treated by Physical Therapist Assistants.
- Assesses resident's needs; identifies and initiates appropriate physical therapy interventions; collaborates cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
- Observes and evaluates treatment affect, accomplishment of short- and long-term goals. Recommends change to physician, if needed
- Performs an appropriate evaluation on each patient and performs reassessments, as per policy. Performs accurate and timely GG scoring. The population is adult and geriatric residents in an SNF setting.
- Able to assess resident pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
- Formulates and updates resident specific plan of care for physical therapy.
- Other duties as assigned.
- Current Florida professional licensure as a Physical Therapist
- Must have long term care experience. Knowledge of PDPM, Section GG and LTC program development.
- Familiar with Net Health and Point Click Care.

- Requires active Florida Professional Physical Therapist license.
