In this role, you won't just coordinate care—you'll be a vital guide for patients and families, helping them navigate complex health, insurance, and social systems to achieve their health goals.
Why Lamoille Health Partners?
- Mission-Driven Work: Make a tangible, daily impact on the health and well-being of our rural community by supporting underserved populations.

- The "Vermont Way of Life": Enjoy predictable scheduling, a supportive culture, and a work-life balance that lets you experience everything our beautiful region has to offer.
- Loan Repayment Eligible: As an FQHC, our site is eligible for various federal and state loan repayment programs (such as NHSC).
- Robust Benefits: Competitive salary, comprehensive health/dental/vision plans, 401(k) with match, and generous Earned Time Off (ETO).
The Impact You'll Make
- Holistic Support & Counseling: Use motivational interviewing and a whole-person approach to assess social, emotional, and financial factors, helping patients build actionable health goals.
- Resource Navigation: Connect patients with essential community resources, including sliding fee scales, low-cost medications, transportation support, and post-hospital discharge care.
- Brief Interventions: Provide brief individual counseling and utilize evidence-based tools (like CALM or CAMS models) to support patients navigating acute distress or barriers to external clinical care.
- Care Coordination: Serve as the bridge between individuals, families, and multidisciplinary providers to ensure seamless communication and continuity of care.
- Quality & Growth: Participate in patient-centered medical home (PCMH) initiatives, team huddles, and quality improvement projects (like PDSA cycles) to continuously enhance our patient care standards.
Who You Are
- Licensed & Qualified: You hold an active professional license or certification aligned with Vermont standards (e.g., mental health counseling, social work, or addictions) and follow best practices outlined by your respective governing board.
- A Compassionate Communicator: You excel at building trust quickly, utilizing motivational interviewing, and practicing culturally sensitive care.
- An Expert Problem Solver: You understand family systems, community resources, insurance complexities, and how to successfully reduce barriers to care.
- A Team Player: You thrive in an interdisciplinary environment, communicate seamlessly in an EMR, and maintain absolute dedication to patient privacy and confidentiality.
