Registered Nurse (RN) – Home Health

Service Area: Monroe to Taylor, Michigan

We are looking for a dependable, skilled, and patient-focused RN to join our growing home health team. This is an excellent opportunity for an RN who values independence, flexibility, quality patient care, and building meaningful relationships with patients and families.

What You’ll Do

As a Home Health RN, you will provide skilled nursing care to patients in their homes while helping them remain safe, independent, and comfortable.

Your responsibilities will include:

• Conducting Start of Care (SOC) visits and completing required OASIS assessments/documentation during the initial visit.

• Performing comprehensive nursing assessments and identifying patients’ skilled nursing needs.

• Developing and implementing individualized plans of care in collaboration with the physician and interdisciplinary team.

• Providing skilled nursing treatments and follow-up visits based on the patient's plan of care.

• Managing and monitoring medications, disease processes, symptoms, wounds, and other skilled nursing needs as ordered.

• Educating patients and caregivers regarding medications, disease management, safety, fall prevention, and appropriate self-care.

• Monitoring patient progress and communicating changes in condition to physicians and the care team.

• Coordinating care with LPNs, PTs, OTs, aides, physicians, caregivers, and other healthcare professionals.

• Opening new cases and, when appropriate, transitioning ongoing treatment visits to an LPN while maintaining continuity of care and clinical oversight.

• Completing timely, accurate, and compliant clinical documentation.

• Recognizing changes in patient condition and taking appropriate action to promote patient safety and positive outcomes.

Service Area

This position will primarily cover patients throughout the Monroe to Taylor area , so reliable transportation and the ability to travel between patient homes are required.

Compensation:

$40 - $80 hourly

Qualifications:

• Current, active RN or LPN license in the state of practice
• Minimum of [1 year] nursing experience (home care experience preferred)
• Strong clinical and assessment skills
• Excellent communication and organizational abilities
• Ability to work independently and manage time effectively
• Valid driver’s license and reliable transportation
