The LPN functions as part of the interdisciplinary health care team. The LPN, Advanced serves as a resource for other team members and is active in the orientation of new staff. May be required to make home visits.Responsibilities:Provides direct and indirect nursing care individualized to the patient's needs.Collects and records relevant patient health data accurately.Evaluates health data to identify patient problems.Implements and documents specified interventions.Identifies deviations from expected patient outcomes.Adheres to approved, research-based practice standards.Directs and supervises patient care provided by other LPNs and support personnel.(LPN Ambulatory Care Advanced) Identifies practice problems and consistently seeks solutions.Performs and documents Point of Care testing and quality control.Maintains and documents testing reagents/supplies and equipment.Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.Delivers care in a non-judgmental and non-discriminatory manner.Assures that patients have informed consent signed for indicated procedures.Maintains current licensure, certification, and competencies.Provides proof of current BLS validation to supervisor.Completes required unit credentialing.Provides to supervisor evidence of completion all required annual training and competency validation.Practices within the New York State-defined LPN scope of practice.Acquires and maintains current knowledge and skills in his/her area of practice.Participates in required number of hours educational activity related to job function.(LPN Ambulatory Care Advanced) Demonstrates operational problem-solving ability.Performs similar or related duties as requested or directed.Performs other duties as requested and observed by supervisor or manager.Education:Graduation from a Practical Nursing program, requiredExperience:Graduate Nurse, requiredPrevious healthcare experience, preferredLicense/Certification:Current NYS licensure as a Practical Nurse or valid interim permit, requiredBLS certification within 6 months of hire, requiredAdditional credentials are determined by specific work unit, requiredSkills:Clinical SkillsCommunicationCritical ThinkingInteracting with PeopleMulti-TaskingReading/Writing/ComprehensionPhysical Requirements:Light Work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects.Occasional balancing, crouching, kneeling, typing/clerical/dexterity, grasping, lifting, pulling, pushing, reaching, stoopingFrequent hearing, seeing/monitor/computer use, standing, talking, visual acuity (color, depth, perception), walkingHazards:Frequent bodily fluids/bloodborne exposureWorking Conditions:Occasional odorsFrequent noiseTravel:Occasional Travel - The job may require travel from time-to-time, but not on a regular basisBassett Healthcare Network's commitment to our employees includes benefit programs carefully designed with the needs and values of all our employees in mind.
