10k sign on bonus (payable 50% after 6 months, 50% after 12 months)

This NP Role requires flexibility to provide coverage throughout Los Angeles and parts of Inland Empire (Inland Empire includes San Bernardino, Riverside, Hemet, Victorville, Moreno Valley, Fontana, Hesperia, Rialto, Ontario, Apple Valley, Perris, Indio etc)

Patient visits are grouped by geographic region, so clinicians typically see patients within the same city or nearby areas on a given day

Nurse Practitioner Opportunity

Join a Nurse Practitioner-led organization focused on delivering high-quality, preventive care to geriatric patients. This role involves completing comprehensive Medicare Annual Health Assessments (AHAs) in patient homes. You will use advanced diagnostic tools and Electronic Medical Records (EMR) to ensure thorough evaluations and collaborative care, directly enhancing patient outcomes without functioning as a Primary Care Provider (PCP).

Job Duties

Patient Care

• Work with geriatric patients
• Complete Medicare Annual Health Assessments at clinics and home visits

Patient Load

• 3–5 patients/day on home visit days (number of days will vary depending on patient load)
• 7-8 patients/day in clinic (this is not daily but occassional)

Clinical Responsibilities

• Perform H&Ps
• Use preventive health screening tools (PAD and Neuropathy, diabetic retinal cameras, A1c and kidney testing for diabetics)
• Navigate and chart via EMR systems
• This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role.

Requirements

• Current NP license for the state
• Ability to use a computer efficiently
• EMR systems and electronic documentation experience
