Nurse Practitioner

Position Summary

The School-Based Health Center (SBHC) Lead Nurse Practitioner serves as both a clinical provider and site leader within Children's Aid's network of school-based health centers serving students from Pre-K through high school. In collaboration with the supervising physician, this role provides comprehensive primary, acute, and reproductive healthcare services while supporting the overall health and wellness goals of the school community.

Reporting to the Assistant Medical Director, the Lead Nurse Practitioner provides day-to-day clinical leadership for the assigned School-Based Health Center, helping ensure high-quality patient care, efficient clinical operations, and compliance with New York State Department of Health (NYS DOH), New York City Department of Health and Mental Hygiene (NYC DOHMH), Medicaid, and Children's Aid policies and procedures. Working closely with the SBHC Program Manager, Community School Director, school leadership, and interdisciplinary partners, this role supports integrated student services and advances accessible, student-centered healthcare.

Essential Duties and Responsibilities

Clinical Care & Patient Services

• Provide comprehensive primary, acute, and reproductive healthcare services to pediatric patients within the School-Based Health Center (SBHC) program in collaboration with the supervising physician.

• Obtain and document patient medical, developmental, and psychosocial histories, and perform physical assessments and screenings in accordance with clinical standards and NYS DOH guidelines.

• Assess patient health status, identify normal and abnormal findings, and recognize deviations in growth and development to support appropriate treatment planning and referrals.

• Order, interpret, and follow up on routine diagnostic tests, screenings, referrals, emergency visits, clinic visits, and hospitalizations, as appropriate.

• Provide treatment for minor illnesses and injuries and consult with the supervising physician when clinically indicated.

• Administer routine immunizations in accordance with NYS DOH standards and maintain site eligibility within the Vaccines for Children Program.

• Provide outpatient follow-up care and guidance to patients and families, including telephone triage and support related to ongoing medical concerns.

• Maintain accurate, timely, and complete medical documentation in accordance with organizational, regulatory, and Medicaid requirements.

• Demonstrate clinical competency in providing age-appropriate care to the student populations served across the SBHC network.

Leadership, Operations & Site Management

• Provide day-to-day clinical and operational leadership for the assigned School-Based Health Center and serve as a clinical resource for staff supporting patient care and service delivery.

• Collaborate with the SBHC Program Manager, supervising physician, administrative staff, Community School Director, and school leadership to support efficient site operations and high-quality patient care.

• Supervise medical clerical staff and contribute to performance evaluation and performance management activities.

• Support site productivity goals and ensure effective coordination of daily clinical operations and workflows.

• Participate in the development, implementation, review, and continuous improvement of clinical protocols, workflows, and quality assurance practices.

• Promote Children's Aid's mission, values, and organizational policies through daily practice and leadership.

Compliance, Quality Assurance & Professional Practice

• Ensure compliance with NYS DOH, NYC DOHMH, HIPAA, HITECH, OSHA, infection control requirements, Medicaid regulations, and Children's Aid policies and procedures.

• Partner with the SBHC Program Manager to support quality assurance activities, emergency preparedness requirements, environmental surveys, audits, and regulatory reporting.

• Maintain required statistical reporting and documentation to support regulatory compliance and operational oversight.

• Participate in rotating after-hours on-call coverage as assigned.

• Maintain current clinical knowledge through continuing education and professional development to support evidence-based practice.

Minimum Qualifications

• Master of Science in Nursing (MSN) from an accredited college or university required.

• Current New York State license as a Registered Nurse and Nurse Practitioner required.

• Board certification as a Pediatric Nurse Practitioner through the Pediatric Nursing Certification Board (PNCB) or another nationally recognized certifying organization required.

• Current New York State Nurse Practitioner certification required.

• Completion of all continuing education requirements necessary to maintain licensure and certification.

• Demonstrated ability to provide comprehensive pediatric primary care in a collaborative clinical environment.

• Demonstrated ability to communicate effectively with children, adolescents, families, school personnel, healthcare professionals, and interdisciplinary teams.

• Ability to work effectively with individuals from a wide range of cultural, social, and economic backgrounds.

• Knowledge of applicable NYS DOH, NYC DOHMH, Medicaid, HIPAA, HITECH, OSHA, and infection control requirements.

• Ability to maintain accurate clinical documentation and manage multiple priorities in a fast-paced healthcare environment.

• Bilingual (Spanish/English) preferred.

• Ability to work onsite in a school-based health center and participate in rotating after-hours on-call coverage.

Compensation Information:
$120000.00 / Annually - $130000.00 / Annually
