Job Description
Title: LPN - Long-Term Care

Schedule: FT Nights (11 PM - 7:30 AM) - 5x8s, Every Weekend, Holiday

Assignment Details:
- Contract Length: 14 Weeks
- Guaranteed Hours: 40 Hours per Week

Requirements:
- NY LPN Licensure
- BLS Certification
- 1 year experience as an LPN in a Long-Term Care Facility

Roles & Responsibilities:
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect and communicate data relevant to residents' physical and psychosocial conditions to the RN and interdisciplinary team.
- Contribute to the creation of culturally relevant care plans reflecting nursing diagnoses and interventions.
- Evaluate resident outcomes and recommend revisions to care plans as necessary.
- Maintain current knowledge and competency in nursing practice through continuing education.
- Collaborate with residents, families, and team members to ensure high-quality care and customer satisfaction.

Compliance / Notes:
- Occupational exposure to bloodborne pathogens is reasonably anticipated, with required tasks involving potential contact with blood, body fluids, or tissues.
