Position Overview

The Occupational Therapist plays a vital role within an interdisciplinary team, focusing on the delivery of therapeutic services to assist patients in regaining their optimal level of function. The goal of these services includes enhancing meaningful daily activities such as self-care, leisure, and work. This position operates under the supervision of the Home Health and Hospice Manager.

Shift Information

Shift: Monday to Friday, 8:00 AM - 4:30 PM, with a weekend rotation. Attendance is required during Thanksgiving and New Year's.

Essential Job Functions

• Evaluate patient functional status and implement the physician's plan of care.
• Conduct assessments and reassessments, making informed decisions based on objective tests and measurements.
• Communicate observations and treatment suggestions effectively as a member of the interdisciplinary team through meetings and individual interactions.
• Provide education and guidance to patients and coworkers as needed.
• Work independently while also collaborating with team members.
• Respect and uphold patient rights while conveying empathy and compassion to patients experiencing grief, pain, or trauma.
• Maintain accurate and concise documentation of all communications, coordination efforts, and treatment activities.
• Adapt to emergency situations and remain observant to any changes in patient conditions.
• Support the mission, vision, values, policies, and procedures of the organization.
• Participate in required education programs as applicable to the position.
• Perform other related duties as assigned.

Qualifications

Education

• Required: Graduate of an occupational therapy curriculum accredited by the American Medical Association and the American Occupational Therapy Association.
• Desired: Evidence of continuing education relevant to the area of practice and skill level.

Licenses and Certifications

• Required: Current Occupational Therapist license issued by the State of Oregon.
• Valid driver's license.
• Current certification in Basic Life Support (BLS).
• Proof of auto insurance.
• Preferred: Registration as an Occupational Therapist with the American Occupational Therapy Association.
• Completion of CRASE training, required if assignment extends beyond six months.
