Registered Nurse (RN) - Case Management

Case Manager – Substance Abuse

Schedule: Wed-Sat 10:00 AM - 6:30 PM

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 72 hours biweekly

Requirements:

• Bachelor's Degree (BA) or equivalent; or Associate's degree in Human Services field
• 2-3 years related experience or training in the area of chemical dependency
• Certified Addiction Counselor (CAC) preferred
• Ability to complete on-site CPI and CPR training/certification

Roles & Responsibilities:

• Maintain a caseload that includes conducting individual intake evaluations and treatment planning
• Complete documentation in compliance with regulations in a timely manner
• Coordinate with collaterals and referral sources as needed
• Assist in the retrieval of patient medications from local pharmacies
• Support staff with milieu management and crisis management
• Coordinate transportation needs for appointments and aftercare
• Engage in identifying and locating contraband and performing unit searches as necessary
• Assess patients' continuing care needs and arrange appropriate programming
• Educate and lead group programming sessions
